Treating plantar fasciitis and treating Achilles tendinopathy are not the same process. What Is Causing Your Heel Pain?

Accurate Diagnosis Is Where Effective Heel Pain Treatment Starts

Heel pain is most commonly felt at the underside of the heel, directly at the front of the heel bone, or at the back of the heel where the Achilles tendon attaches. The location and character of the pain, whether sharp or aching, worse in the morning or after activity, at the bottom or back of the heel, provide important diagnostic information before any examination begins.

At your first appointment, your podiatrist will take a thorough history, perform a physical examination of the heel and surrounding structures, assess your gait mechanics, and order imaging where indicated. X-rays identify heel spurs, bone abnormalities, and stress fractures. Diagnostic ultrasound can visualize the plantar fascia and Achilles tendon directly to assess thickness, tearing, or degeneration. This evaluation is what distinguishes a clinical diagnosis from a guess.

Plantar fasciitis is the most common cause of heel pain in adults, responsible for approximately 2 million podiatry visits in the United States each year. It develops when the plantar fascia, the connective tissue band connecting the heel bone to the base of the toes, sustains repetitive microtrauma at its calcaneal insertion, producing a cycle of incomplete healing and chronic inflammation.

The hallmark symptom is sharp pain with the first steps of the morning that eases after 5 to 10 minutes of walking, then returns after prolonged activity or standing. Localized tenderness at the underside of the heel, toward the inner edge, is the key physical finding.

Conservative treatment is successful in 80 to 90 percent of patients and includes a structured stretching and strengthening program, custom orthotics, activity modification, anti-inflammatory management, and in some cases corticosteroid injection or shockwave therapy. Surgical plantar fascia release is reserved for cases that do not respond to at least 6 months of appropriate conservative care.

A heel spur is a calcium deposit that forms on the underside of the heel bone, typically at the attachment of the plantar fascia. Heel spurs develop in response to chronic tension and microtrauma at the calcaneal insertion, the same process that causes plantar fasciitis, and are present in approximately 70 percent of plantar fasciitis cases on imaging.

Importantly, heel spurs are not always painful in themselves. Many patients with heel spurs on X-ray have no symptoms, and many patients with plantar fasciitis have no visible spur. The pain is almost always due to the surrounding plantar fascial inflammation rather than the bony deposit itself. Treatment targets the inflammation and biomechanical cause, not the spur directly.

Surgical removal of a heel spur is rarely required and is considered only when a prominent spur is confirmed to be a direct mechanical source of pain resistant to all conservative interventions.

Achilles tendinopathy is the degeneration of the Achilles tendon, the large tendon connecting the calf muscles to the back of the heel bone. It produces pain and stiffness at the back of the heel and lower calf, typically worse with the first steps in the morning and after exercise.

There are two distinct presentations: insertional Achilles tendinopathy, where degeneration occurs at the tendon’s attachment to the heel bone, and mid-portion tendinopathy, where it occurs 2 to 6 cm above the insertion. These require different treatment approaches; clinical examination and imaging are important before beginning rehabilitation.

Conservative management includes eccentric and isometric loading protocols, footwear modification, heel lifts, custom orthotics, and, where indicated, extracorporeal shockwave therapy. PRP injection and surgical debridement are advanced options for resistant cases.

The retrocalcaneal bursa is a small fluid-filled sac between the Achilles tendon and the back of the heel bone. When inflamed, typically from repetitive pressure from footwear or overuse, it produces pain, swelling, and tenderness at the very back of the heel, just above the heel counter of the shoe.

It is frequently associated with Haglund’s deformity and is common in runners, cyclists, and patients who wear stiff-backed footwear. Conservative treatment includes rest, ice, anti-inflammatory management, heel lifts, and footwear modification. Corticosteroid injection into the bursa provides effective short-term relief in many cases.

Haglund’s deformity, also called the “pump bump,” is an enlargement of the bony prominence at the back of the heel. It develops in patients with high arches, tight Achilles tendons, or a walking pattern that concentrates pressure on the posterior heel, and is frequently aggravated by rigid-backed footwear.

Diagnosis is based on clinical examination and confirmed with X-ray. Conservative treatment includes footwear modification, heel lifts, padding, calf stretching, and anti-inflammatory management. Surgical correction, resection of the bony prominence, is considered when conservative care over 6 months has not provided adequate relief.

A stress fracture of the heel bone is an overuse injury most commonly seen in runners who rapidly increase training volume, military recruits, and patients with reduced bone density. It produces deep, diffuse heel pain that worsens progressively with activity and does not follow the morning-pattern characteristic of plantar fasciitis.

Stress fractures are not always visible on initial X-ray; MRI is the most sensitive imaging modality and is ordered when clinical suspicion is high. Treatment involves relative rest and non-weight-bearing or protected weight-bearing for 6 to 8 weeks, with a graduated return to activity thereafter.

If you develop sudden, severe heel pain that makes weight-bearing impossible, seek a same-day medical evaluation. This may indicate a stress fracture or an acute plantar fascia tear.

Tarsal tunnel syndrome is a nerve entrapment condition in which the tibial nerve is compressed as it passes through the tarsal tunnel — a narrow passage on the inner side of the ankle. It produces burning, tingling, or numbness in the heel and arch rather than the sharp, mechanical pain of plantar fasciitis, and is often worse at night or after prolonged standing.

Diagnosis requires a thorough neurological examination and may include nerve conduction studies. Conservative management includes orthotics to reduce nerve tension, anti-inflammatory medication, and corticosteroid injection around the nerve. Surgical decompression is an option when nerve entrapment is confirmed and conservative management has not provided relief.

Sever’s disease is the most common cause of heel pain in children aged 8 to 14, occurring during periods of rapid bone growth when the Achilles tendon becomes relatively tight against the developing heel bone. It produces pain in the back and bottom of the heel during and after physical activity.

It is a self-limiting condition that resolves with growth plate closure, but it can be significantly painful during active phases. Management includes activity modification, heel lifts, calf stretching, and, in some cases, custom orthotics to reduce mechanical tension at the growth plate. Rest from high-impact sport is often necessary for a defined period.

The most common reason heel pain becomes chronic is that treatment begins before the diagnosis is confirmed. Plantar fasciitis and Achilles tendinopathy look similar in the early stages but require different rehabilitation approaches. Treating Achilles tendinopathy with plantar fascia-focused stretching alone, or prescribing orthotics for a nerve entrapment condition, does not resolve the underlying cause — and months of misdirected treatment delay recovery.

A Diagnosis at Your First Visit – Not Just a Referral

Your first appointment is a diagnostic visit, not a treatment session. Your podiatrist will take a full history of your symptoms, when the pain started, what makes it better or worse, whether it is worst in the morning, and what activity level you are trying to return to. This information guides the examination before your foot is ever touched.

After the history, your podiatrist will examine the heel and surrounding structures, assess your range of motion and gait, and order imaging where indicated. In most cases, X-rays are taken at the same appointment. Diagnostic ultrasound of the plantar fascia or Achilles tendon can be performed in-office and provides real-time information that changes treatment decisions.

Before you leave, you will have a confirmed or working diagnosis, a treatment plan, and clear guidance on what to do before your follow-up. Most heel pain conditions require a structured course of treatment, not a single visit, and your podiatrist will set expectations around timeline and milestones at this first appointment.

Morning heel pain that is sharpest with the first steps and eases after several minutes of walking is the classic presentation of plantar fasciitis. During sleep, the plantar fascia shortens. When you first load your foot, the tissue is abruptly stretched before it has had a chance to adapt to weight-bearing. The same pattern, pain after rest that eases with movement, can occur after sitting for long periods during the day. If this pattern is consistent, a podiatric evaluation is warranted.

This depends entirely on the diagnosis and how long the condition has been present before treatment begins. Acute plantar fasciitis caught early typically shows meaningful improvement within 6 to 8 weeks. Chronic plantar fasciitis present for more than 6 months may take 3 to 6 months of structured treatment. Achilles tendinopathy typically requires 3 to 6 months of graded loading rehabilitation. Acting on early symptoms is the most effective way to reduce recovery time.

The large majority of heel pain conditions respond to conservative management and do not require surgery. For plantar fasciitis, studies show that 80 to 90 percent of patients achieve resolution with non-surgical treatment. Surgery is considered only after an appropriate trial of conservative care, typically 6 months for plantar fasciitis, has not produced adequate improvement. If your podiatrist recommends surgical evaluation, they will explain the specific indication, procedure, and expected outcomes at your appointment.

Running is a significant risk factor for plantar fasciitis, Achilles tendinopathy, and calcaneal stress fractures because it places repetitive high-load demand on the heel structures. Increasing training volume too rapidly, running in worn footwear, and biomechanical factors such as overpronation or restricted ankle dorsiflexion all increase risk. Most runners can be treated conservatively and return to full activity with appropriate management and biomechanical correction.

In most cases, yes. Conservative heel pain management is designed to be compatible with normal daily activity for patients without physically demanding occupations. Patients in jobs involving prolonged standing or heavy physical labor may require a period of modified activity. Your podiatrist will advise on activity modification appropriate to your specific diagnosis, job demands, and condition severity at your evaluation.
