Heel pain is common. Plantar fasciitis is the most common cause of it, accounting for the majority of heel pain presentations seen in podiatric practice. But common does not mean easy to self-identify. Patients arrive at our New Jersey offices having attributed their heel pain to aging, overexertion, or a bad pair of shoes, often for months before recognizing that what they have is a specific, diagnosable condition with a specific, treatable cause.

This post is about the full symptom picture of plantar fasciitis: what it feels like across each stage of the condition, where the pain is located, how the pattern behaves throughout the day, and how to distinguish it from other causes of heel pain that present similarly.

What Plantar Fasciitis Actually Feels Like

Plantar fasciitis has a recognizable symptom pattern that distinguishes it from most other causes of heel pain. An experienced podiatrist can often make a clinical diagnosis from history alone, before any imaging, because the pattern is that specific.

Morning Heel Pain and First-Step Pain

The signature symptom is sharp, localized pain at the bottom of the heel felt most intensely with the first steps after waking or after any extended period of rest. This is clinically referred to as post-static dyskinesia.

The mechanism: the plantar fascia contracts into a shortened position during sleep. Any existing micro-damage in the tissue begins to stabilize overnight in that contracted state. The moment full body weight is applied on standing, the fascia is suddenly loaded, and the damaged fibers are re-stressed all at once. The result is immediate, sharp pain, typically located at or just in front of the heel bone toward the inner edge of the foot.

What makes this pattern clinically distinctive is that it eases with movement. After 10 to 20 minutes of walking, the tissue warms, circulation increases, and the sharp pain subsides to a dull ache or nearly disappears. This temporary improvement is the detail that most often delays diagnosis. Patients interpret the improvement as evidence that nothing serious is wrong. As the condition progresses, the improvement window shortens, and the pain at rest becomes more persistent. At the early stages, the pattern of improvement with movement is one of the clearest diagnostic indicators available.

Pain After Prolonged Sitting or Inactivity

The second most reliable symptom pattern is pain that returns after sitting for an extended period and then standing again. After a long car ride, a desk-based afternoon, or even sitting through a film, patients stand and feel the same sharp heel pain as the morning, though typically slightly less severe.

This recurrence after any sustained inactivity reflects the same mechanism as morning pain. The tissue contracts when unloaded and is re-stressed when weight is reapplied. If your heel consistently hurts more when you first stand than during the activity that follows, this is a meaningful diagnostic pattern worth documenting before your appointment.

End-of-Day Pain and Pain After Activity

As plantar fasciitis progresses or in more established presentations, the symptom pattern broadens. What began as morning pain alone now appears at the end of a long day of standing or walking. The cumulative loading of the tissue over hours catches up by late afternoon or evening.

Runners and active patients often notice that the pain intensifies in the hours after exercise rather than during it. A long run may feel manageable, but the heel becomes significantly painful by that evening or the following morning. This post-activity pattern is also characteristic of plantar fasciitis and distinguishes it from conditions in which pain is worst during loading rather than after.

Arch Tightness and Stiffness

Many patients with plantar fasciitis report tightness or stiffness through the arch of the foot, particularly in the morning. This reflects involvement of the plantar fascia along its full length rather than only at the heel insertion. Pulling or tension from the heel toward the ball of the foot, with the toes flexed upward, is a useful self-test that many patients find reliably reproduces their symptoms.

Where Exactly Does It Hurt?

Location matters for diagnosis. Plantar fasciitis pain is almost always felt in one of these areas:

  • At the very bottom of the heel, specifically the inner (medial) aspect of the heel bone
  • Just in front of the heel, along the origin of the plantar fascia
  • Occasionally radiating into the arch, but not typically extending into the ankle or up the leg

When you press firmly on the inner edge of your heel with your thumb, plantar fasciitis will usually produce a distinct point of tenderness. This is the medial calcaneal tuberosity, where the plantar fascia attaches to the heel bone and where the majority of cases originate.

Pain that is primarily at the back of the heel rather than the bottom is more likely to involve the Achilles tendon. Pain that wraps around the ankle or radiates into the toes may suggest nerve involvement. Location is one of the clearest ways to begin distinguishing plantar fasciitis from other conditions before a clinical evaluation.

Plantar Fasciitis vs. Other Causes of Heel Pain

Not all heel pain is plantar fasciitis, and misidentifying the cause leads to ineffective treatment. Here is how plantar fasciitis compares to the conditions most commonly confused with it.

Plantar Fasciitis vs. Heel Spur

A heel spur is a calcium deposit on the underside of the heel bone. Roughly half of all plantar fasciitis patients have a heel spur visible on X-ray, which leads many people to assume the spur is causing their pain. It is not.

The spur itself is largely asymptomatic. The pain comes from the inflamed plantar fascia tissue. People with heel spurs and no plantar fasciitis often have zero pain. Treatment targets the fascia, not the spur, and resolving the fasciitis resolves the pain regardless of whether a spur is present.

Plantar Fasciitis vs. Achilles Tendinopathy

Achilles tendinopathy involves the tendon attaching the calf muscles to the back of the heel bone. It produces pain at the back of the heel or just above it, anatomically distinct from the bottom-of-heel pain of plantar fasciitis.

Achilles pain is also often worse in the morning and at the start of activity, but the location is different. Pressing on the Achilles tendon or the back of the heel reproduces the pain, not the bottom of the heel. Both conditions are aggravated by tight calf muscles and can coexist in the same patient.

Plantar Fasciitis vs. Tarsal Tunnel Syndrome

Tarsal tunnel syndrome involves compression of the posterior tibial nerve as it passes through a narrow channel on the inside of the ankle. It can produce pain, burning, tingling, or numbness along the bottom of the foot and into the heel.

The distinguishing features are nerve symptoms, including tingling, burning, and numbness, rather than purely mechanical pain, and the fact that symptoms may be reproduced by tapping over the nerve at the inside of the ankle. Plantar fasciitis typically does not cause tingling or numbness.

Plantar Fasciitis vs. Stress Fracture

A calcaneal stress fracture can closely mimic plantar fasciitis in location and intensity. The difference is in pain behavior. Stress fracture pain tends to be more constant, present with any weight-bearing, and does not show the characteristic improvement with movement that plantar fasciitis does. Squeezing the heel from both sides simultaneously often produces pain in stress fractures. Imaging is required to rule this out, particularly in runners or individuals who have recently increased activity significantly.

  • Heel pain severe enough to change the way you walk
  • Pain that is getting progressively worse over days
  • Swelling, bruising, or visible deformity around the heel following any injury
  • Tingling, numbness, or burning sensations alongside the heel pain
  • Pain in both heels simultaneously
  • You have diabetes, peripheral neuropathy, or poor circulation and are experiencing any new foot or heel pain
